How to Register ?

  1. Visit www.anantent.com/feedback_login.php
  2. Click on ‘Create Account’.
  3. Log in to your account.
  4. Select Language. (You can make the questionnaire in multiple languages)
  5. Select a predesigned Questionnaire in ‘Standard Questionnaire’ or add questions of your own in ‘My Questionnaire’.
  6. You will get the QR Code via mail.
  7. Display the QR code at prominent points in your hospital.
  8. The patient can scan the QR code and leave a feedback.
  9. You can access the Questionnaire details by logging in to your account.
How to Use ?

  1. Open the QR Code scanning app in your smart phone.
  2. Scan the code.
  3. Fill in your details.
  4. Answer the questionnaire to give your feedback.
  5. You will get a feedback on your email id.
